Regular Appointment

The following appointment options are available:

  • Follow-up (in-person)
  • Follow-up (phone)

Note: Select these options for new health concerns, review/discuss test results, medication refills, treatment adjustments, etc.

  • Physical

Note: Limited to 1 per year, routine visit with your provider to evaluate your overall health.

  • Form Completion

Note:  These appointments are 1-hour in length. More (or less) time may be required to complete your form. This will be at your provider’s discretion. Please include the type/name of form in the “Reason for Visit”.

Urgent Appointment

The following appointment options are available:

  • Urgent (in-person)

Note:  Urgent appointments are limited and are reserved for health concerns that require attention within 24 to 36 hours (cough, cold, flu, fever, Covid, rash, new injury, UTI).
