Note: Select these options for new health concerns, review/discuss test results, medication refills, treatment adjustments, etc.

  • Physical (Periodic Health Exam and/or Cervical Screening)

Note: Limited to 1 per year, routine visit with your provider to evaluate your overall health.

The following are not available to book online.  Please call the office to book:

  • Forms
  • Procedures
  • Well baby visit
  • Nursing injections

The following appointment options are available:

Note:  Urgent appointments are limited and are reserved for health concerns that require attention within 24 to 36 hours (cough, cold, flu, fever, Covid, rash, new injury, UTI).
