The LLPS is a provincial initiative aimed at reducing avoidable, non-traumatic limb amputations caused by vascular disease and diabetes. The Windsor Team Care Centre is participating in this initiative and has established the Lower Limb Wound Prevention & Treatment Clinic offering comprehensive care, including wound management, personalized treatment plans, and educational resources to support lower limb health.
The Lower Limb Wound Prevention & Treatment Clinic is designed for individuals at high risk of limb amputations, particularly those dealing with complications from Diabetes and Vascular Disease. This includes:
When you visit the Lower Limb Wound Prevention & Treatment Clinic, you can expect:
Primary Care Providers can refer patients to the Lower Limb Wound Prevention & Treatment Clinic using our dedicated referral form. Our aim is to see and assess referred clients within 72 hours of receipt of a referral.
For clients without a primary care provider or are who are experiencing homelessness, can attend the Shelter Health walk in clinics where they may receive a virtual consultation with our Nurse Practitioner.
On April 8, 2025, the Windsor Family Health Team, in partnership with Windsor Regional Hospital and the Windsor Essex Ontario Health Team hosted an Ontario-wide accredited education session for physicians and healthcare professionals. This Continuing Medical Education program focused on providing clinicians with essential strategies, best practices and pathways to address preventative and urgent care for lower limb health.
Topics included: assessing, diagnosing and treating Diabetic Foot Ulcers, Peripheral Arterial Disease, Ischemia, Osteomyelitis, and more.

The Windsor Family Health Team will close at 12 p.m. on December 24th, and remain closed December 25th and 26th. We will reopen December 29th and 30th from 8:30 a.m. to 4:30 p.m., and close again at 12 p.m. on December 31st.
Regular hours resume on January 2nd.
Please note that medication refill requests take 3-5 business days to be completed. Remember to check to your medications and renewals and plan ahead to ensure that you have what you need to continue your medication over the holidays.
