The Family Practice Program offers primary health care to residents of all ages living in Windsor and the surrounding area. An interprofessional group of health care practitioners, including family physicians and nurse practitioners provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs and to manage health concerns that are episodic and chronic in nature. The focus of the health team is on preventative health care, disease management, rehabilitation, palliative care and health promotion. All programs and services are grounded in the Model of Health and Wellbeing.
