For enrolled patients who have been diagnosed with atrial fibrillation, this program offers monitoring, the affects atrial fibrillation has on daily life in partnership with the Asthma Research Group Windsor-Essex. Patients learn how to recognize symptoms and how to respond appropriately. Medication reviews, the benefits of regular exercise, healthy eating, how to cope with atrial fibrillation along with referrals to other supports are available.
A referral is needed from a Windsor Family Health Team physician or nurse practitioner to join the Heart Health Program.
