We are a primary health care clinic serving patients in Windsor, Ontario and the surrounding area. Made up of a team of interprofessional health care practitioners, we strive to provide our patients with the highest quality primary health care available.
As a patient of the Windsor Family Health Team (WFHT), you can expect to receive quality, coordinated care focused on helping meet and maintain your health and wellness goals. To achieve those expectations, our team of professionals from various medical fields work with you and each other when needed, to discuss, plan, and implement steps to get you closer to your goals and help you stay on track.
We are NOT a walk-in clinic. All services are available by appointment only.


In 2014, The Windsor Family Health Team adopted a Model of Health and Wellbeing. All of the services and programs that we offer are grounded in this model to ensure that we are meeting the needs of the community that we serve.
Model of Health and Wellbeing defines health in the same way as the World Health Organization:
“a state of complete physical, mental, and social well-being and not merely the absence of disease or infirmity.”
Our goal is to champion health for our patients in the holistic manner of the Model of Health and Wellbeing, especially considering people and communities facing barriers to health.
The Windsor Family Health Team (WFHT) works to deliver high quality health services. Our updated 2023-2027 strategic plan maps out how we are doing this now and into the future. WFHT clients, staff and board of directors worked together to choose the plan priorities. These support our mission, vision and long-term goals. The Windsor Family Health Team has fully embraced and endorsed the Alliance for Healthier Communities Health Equity Charter. As an organization, we recognize the need for health equity and believe this charter provides a roadmap towards achieving this goal.
The Health Equity Charter states, “Access to the highest attainable standard of health is a fundamental human right. Everyone deserves equal access to a full, vibrant life, which is essential to a healthy and just society.” We believe that by adopting this charter, we are committing to providing equitable care to all of our patients. We are dedicated to implementing the principles of the charter in our practice and ensuring that our patients receive the care they need to achieve optimal health outcomes. Together, we can work towards a healthier and more equitable community.

We acknowledge the land and the surrounding waters for sustaining us and we are committed to protecting and restoring these lands and waters from environmental degradation.
The Windsor Family Health Team will close at 12 p.m. on December 24th, and remain closed December 25th and 26th. We will reopen December 29th and 30th from 8:30 a.m. to 4:30 p.m., and close again at 12 p.m. on December 31st.
Regular hours resume on January 2nd.
Please note that medication refill requests take 3-5 business days to be completed. Remember to check to your medications and renewals and plan ahead to ensure that you have what you need to continue your medication over the holidays.
