The Health Promoter is a Registered Practical Nurse with extensive experience working in primary care supporting illness prevention and improved access to care through program development and facilitation. Our health promoter encompasses a broad range of skills to effectively work independently and proactively with patients of the Windsor Family Health Team.
Health Promotion is delivered in a variety of ways by creating programming and activities to engaging in community outreach.

At the Windsor Family Health Team, we offer Social Prescribing as a way to address the social determinants of health. We connect our patients with non-clinical services in the community to improve their health, mental health and overall well-being.
Enrolled patients can get a referral from their primary care provider to see our Health Promoter for a one-on-one appointment to complete a Social Prescribing assessment. If you’re an enrolled patient and would like to self-refer please call the Windsor Family Health Team at 519-250-5656 ext. 205.

The Windsor Family Health Team will close at 12 p.m. on December 24th, and remain closed December 25th and 26th. We will reopen December 29th and 30th from 8:30 a.m. to 4:30 p.m., and close again at 12 p.m. on December 31st.
Regular hours resume on January 2nd.
Please note that medication refill requests take 3-5 business days to be completed. Remember to check to your medications and renewals and plan ahead to ensure that you have what you need to continue your medication over the holidays.
