Counselling with notepad

Registered clinicians with a Master’s Degree in Social Work offer evidence based, multi-model counseling tailored to meet the patient’s individual goals. This time limited individual counselling is available to patients currently enrolled at the WFHT who are 16 years and older.

A referral is needed from a Windsor Family Health Team physician or nurse practitioner to access individual counselling.

*There is currently a waitlist for Social Work appointments. Once your or your primary care provider have completed a referral to Social Work, your name will be added to the waitlist. You will be contacted by telephone to schedule your first appointment as an opening becomes available.*

Group Workshops

Social Worker’s offering evidence-based workshops in collaboration with other team members such as the Health Promoter or Dietician, as well as in partnership with other community organizations. These workshops are offered throughout the calendar year, online or in person and are free for the general public to attend. Please check our community calendar for upcoming event.

Crisis and Community Tools

Community Crisis Centre

Hôtel-Dieu Grace Healthcare’s (HDGH) Community Crisis Centre of Windsor-Essex County works in partnership with community social agencies to provide crisis response services to individuals 16 years of age and older who are experiencing an acute mental illness and/or psychosocial crisis.

Crisis Text Line

  • Free 24-hour support for individuals in crisis. A live, trained crisis responder will be on the other end receiving your texts.

Hope by CAMH

Hope is a free smartphone app that offers suicide prevention information, tools and crisis resources to support individuals when they are experiencing thoughts of suicide. Individuals can create a personalized suicide safety plan.
