ATTENTION PATIENTS

You may experience long wait times when calling the office due to a staffing shortage and a high volume of calls.

  • CLICK HERE to book your appointment online using Ocean. All you need is your health card.
  • If you need a medication refill please call your pharmacy and ask them to fax a Prescription Renewal Request to 519-250-5864. We will not give prescription refills over the phone and require a minimum of 5 business days to respond to your pharmacy.

Digital Solutions

We are proud to offer our patients access to a variety of digital tools to allow you to be more connected with your health care. These tools allow us to offer faster, more accessible appointment booking and management and also gives you the ability to book a virtual visit which you can attend from the comfort of your own home.

To allow us to offer these services, our patients must consent to the use of their email address for communication regarding our digital services including appointment reminders, referral status updates, health promotion/education information, general announcements, and patient satisfaction surveys. Patient Compliments and Complaints

We are committed to listening, learning, and responding to your concerns as we strive to provide the highest quality health care in a welcoming and supportive environment. We are here for you and want you to be satisfied with your care.

If you are an existing patient and have a compliment or complaint you’d like to make us aware of, please download our Patient Compliments and Complaints form and submit it to our office by following the instructions on page one (1) of the form.

Uninsured Services

Although the Windsor Family Health Team provides many free services which are funded by the Ministry of Health, there are uninsured services that will require payment by patients.
