Patient Rights and Responsibilities

As a patient of the Windsor Family Health Team, you can expect your rights as a patient to be honoured and protected. Additionally, we believe that all of our patients have a list of responsibilities related to their role as a patient.

You have the right to:

  • Be treated with consideration, courtesy and respect in a way that fully recognizes your dignity, individuality and cultural background.
  • Privacy and confidentiality of your personal health information.
  • Reasonable accommodation in accordance with the law.
  • Access your health information as directed by legislation.
  • An explanation of your health, treatment and tests including benefits, risks and alternative options.
  • Actively participate in decisions about your care.
  • Refuse treatment and understand the consequences of refusing treatment.
  • Express your concerns through the patient satisfaction survey, comment cards or by speaking directly to a WFHT member.

You have the responsibility to:

  • Be considerate and respectful of any person on site at the Windsor Family Health Team. The WFHT has zero tolerance for the use of profanity, aggressive, violent and disrespectful behaviour towards any staff or visitor of the WFHT. Violation of this responsibility will result in the termination of your care with the WFHT.
  • Give us accurate information about your health, including prescription, non-prescription and recreational substance use.
  • Be responsible for the management of your own medication refills by allowing the WFHT five (5) business days to process requests.
  • Be reliable in attending appointments and give adequate notice to cancel or reschedule.
  • Recognize that appointments are time limited and you may meed to schedule a second appointment in order to have all health concerns addressed.
  • Tell us about any changes to your health card and contact information.
  • Request permission BEFORE using any type of recording device during your appointment.

Uninsured Services

Although the Windsor Family Health Team provides many free services which are funded by the Ministry of Health, there are uninsured services that will require payment by patients.

Sociodemographic Data Collection

Information collected will be used to identify who we serve in the community, and to promote equitable care. Equitable care incorporates unique needs that people may have based on language, income, gender, etc. Data collected is confidential and you may “prefer not to answer” for any question. If you choose to not answer, your care or access to services will not be affected.

After-Hours Care

Our After-Hours Clinic provides timely care for urgent medical concerns that cannot wait for a regular appointment but are not emergencies. This service is available to enrolled patients only. We are open Monday through Thursday from 5:00 p.m. to 8:00 p.m., and on select Saturday mornings (by appointment only – we are unable to accommodate walk-ins).

To request an urgent appointment, please call our office and speak with our team. Urgent concerns may include issues such as coughs, colds, flu symptoms, fevers, new injuries, or urinary tract infections, or other problems that should be assessed within 24–48 hours.

Please note that we are closed on all statutory holidays, as well as Remembrance Day and the second Monday in June. During these times, or when our schedule is full, patients should seek care from an alternative urgent care centre.

Patients may also access:

  • Virtual Care Clinics through Urgent Care Ontario, which provide same-day virtual appointments with licensed healthcare providers for non-emergency issues using an Ontario health card.
  • Health 811, which connects you with a registered nurse day or night for free, and provides secure and confidential health advise.

Our After-Hours Clinic is not an emergency service.
